I have created a form for a silent auction (for a non-profit) and it is flagged at phishing. It only asks for name/phone/email.

I am creating a Form for parents to participate in PTO silent auction and it is getting flagged for phishing. It says: This form can't be distributed as it is asking for personal or sensitive information." We are not asking for anything more then name and phone. Checks would have to be made to PTO. No credit information would be stored in the form. Why is this flagging and how do I stop it?
